Job Summary

Direct Care Workers are responsible for providing in-home personal care and daily living tasks as outlined by an Individual Service Plan of individuals receiving home care services. Non-medical personal care may include assistance with eating, bathing, dressing, personal hygiene, general household activities and/or chores such as light housekeeping tasks, preparing meals, grocery shopping, and laundry.

Job Responsibilities

  • Wellness services to enable adequate nutrition, exercise, the keeping of medical appointments and all other health regimens related to healthy living activities.

  • Chore services needed to maintain the home in a clean, sanitary and safe environment, such as washing floors, windows and walls and tacking down loose rugs and tiles.

  • Each case has specific needs and services which will be discussed on-site with the Consumer Account Manager and the consumer or consumer’s family.

  • Observation and reporting of the consumer’s condition to the Agency.

  • Professional and confidential communication with the Agency and the consumer’s family.

  • Other duties as assigned by a supervisor.

  • Must be able to lift or transfer at least 30lbs.

Qualifications

  • Skills in providing services for consumers with a positive approach.

  • Ability to work as part of a team and positively represent the Agency in the community.

  • Must have patience, good communication skills, math, reading and writing skills

  • Adherence to a professional code of ethics and confidentiality, recognizing the rights of consumers.

  • Knowledge of and ability to comply with the Federal, State, and Agency Mandated Regulations.​

  • High School Diploma or GED preferred

  • Spanish Speaking is a plus
